Job Description

Job Overview We are seeking a highly motivated Medical Assistant/ Front Desk - Quality & Care Gaps to join a busy medical management and consulting company in Wellington. The detail-oriented Medical Assistant/ Front Desk - Quality & Care Gaps will support population health and value-based care initiatives. This in person role helps improve quality outcomes by closing HEDIS and care gaps, supporting provider and patient outreach, maintaining accurate documentation, and assisting with quality reporting and compliance activities. The ideal candidate is organized, collaborative, and comfortable working across teams to help improve patient outcomes and support performance goals. Job Responsibilities Identify, track, and support closure of HEDIS and care gaps for assigned patient populations Review payer reports and quality rosters to identify needed preventive and chronic care services Coordinate patient outreach through phone, text, letters, portals, and EMR reminders Support providers and care teams with documentation, quality reporting, and gap closure efforts Generate and maintain reports related to quality performance, compliance, and outreach outcomes Collaborate with internal teams and payer partners to support value-based care initiatives Qualifications High school diploma or equivalent required At least 2 years of business office experience required; medical office experience preferred Strong customer service, organization, time management, and multitasking skills Ability to work under pressure while maintaining professionalism and patient confidentiality Good communication, interpersonal, and problem-solving skills Working knowledge of medical terminology and human anatomy preferred Ability to work in a fast paced office enviroment
Pay:
From $18.00 per hour
