Job Title:
Clinical Wellness Liaison (CMA)
  • Physician Practice Schedule:
    Monday
  • Friday, 8:00 AM
  • 4:00 PM (No Nights / No Weekends)
Location:
Overland Park, KS (Primary facility base with limited travel within a 10-mile radius) About the Practice KC Medical Care is a small, physician-led medical practice focused on the geriatric population in Independent Living settings. We are not a large corporate healthcare chain. Our practice is built on a direct-access model where quality of coordination is prioritized over high patient volume. The Opportunity KC Medical Care is seeking a professional, organized, and proactive Certified Medical Assistant (CMA) to serve as a Clinical Wellness Liaison. This is a specialized role focused on care coordination, resident advocacy, and clinical support within an Independent Living environment. This position is ideal for a CMA who excels at clinical documentation, patient interaction, and working closely with a dedicated team of Physicians and Nurse Practitioners. Why This Role?

Clinical Focus:
Spend your time on quality patient assessments and care coordination rather than high-volume administrative tasks.
Core Responsibilities Clinical Preparation:
Conduct patient intake, monitor vital signs, and perform "pre-round" reviews to ensure Physicians and NPs have accurate data for their visits.
Transition Management:
Assist in the post-discharge process by organizing hospital/rehab paperwork and medication lists to ensure a safe transition back to the facility.
Patient Navigation:
Act as a liaison for residents, assisting them with communication between their various medical providers and specialists.
Program Support:
Assist in the implementation and daily monitoring of our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
Health Coaching:
Provide education to residents and families on topics such as chronic disease management, medication adherence, and geriatric wellness.
Required Skills & Qualifications CMA Certification:
Must have a valid Medical Assistant certification or equivalent training.
Clinical Accuracy:
Strong knowledge of medical terminology, anatomy, and geriatric care.
Documentation:
Proficiency with EHR/EMR systems (experience with Tebra or PointClickCare is a plus).
Communication:
Must be able to interact positively with residents, families, and facility leadership with a high degree of professionalism.
Organization:
Ability to manage multiple tasks, triage patient needs, and work independently.
Reliability:
Must have a valid driver's license and reliable transportation for travel within a 10-mile radius.
