Job Description
POSITION OVERVIEW
Cross Valley Health & Medicine is an independently owned multi-specialty primary care practice providing medical services to New York State residents. Our medical service line includes internal medicine, addiction medicine, psychiatry, medical cannabis certification, behavioral health, and weight loss management. We are seeking a highly communicative and friendly Nurse Practitioner to encounter patients at our rapidly growing Newburgh office. The desired Nurse Practitioner will provide treatment, diagnosis, and consultations to our patients, along with performing annual wellness examinations. The Nurse Practitioner will work in a team-based environment with clinical support staff. The core services the Nurse Practitioner will be providing are chronic disease management, preventive care, and weight loss management. The practice is dedicated to improving the physical and mental health of our patients, so appropriate medical care rendered at the experience level of the applicant will be expected.RESPONSIBILITIES
Work in collaboration with physician and other clinical staff members to provide health care across the continuum of care to assigned patients Obtain patient health history to perform a comprehensive health assessment and conduct a wellness examination Treat patients for various medical conditions that involve the management of chronic diseases Refer patients to appropriate specialist(s) as medically applicable Prescribe and/or administer medications/vaccinations as medically applicable Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances Complete all documentation in EMR in a timely fashion. Participate in a quality assurance review periodically, including a systematic review of medical records and treatment plans Treat patients and their families with respect and dignity Perform all other clinical/administrative duties as assignedQUALIFICATIONS
Education:
Graduate from an accredited post-graduate nursing institutionLicensure:
Must be able to practice advanced nursing unrestricted in New York State. Actively hold a NYS controlled DEA registration number. Not be barred from participating with governmental insurance plans.Experience:
3-5 years post-graduate experience as a Nurse Practitioner. Internal Medicine experience is a plus.Certification:
Holder of an active BLS certificationLanguage:
Candidate must speak in English & Spanish at a conversational level as we have more than 50% of patients that speak Spanish.Job Type:
