Role:
We are hiring a bilingual Medical Technician to support our Remote Patient Monitoring (RPM) and assist with Chronic Care Management (CCM) services in our partner clinics. This is a full-time, 1099 contractor role where you'll onboard patients to RPM in person, ensure they stay compliant, and serve as a liaison between patients, providers, and our care center team that handles CCM calls and care planning.
Key Responsibilities:
Remote Patient Monitoring (RPM): Identify eligible patients using the daily clinic schedule and diagnosis codes Download the RPM app to the patient's phone and connect their Bluetooth blood pressure monitor Educate patients on how to take their readings (goal: 4x/week) Monitor daily compliance and follow up with patients who are missing readings Communicate abnormal readings to the clinic staff Chronic Care Management (CCM)
Support:
Educate patients in-person about the CCM program while they are in the clinic Obtain signed consent forms and explain what to expect from the care center Follow up with patients who are not answering the care center's calls Remind patients that the care team works directly with their provider and may call from a specific number If a patient answers the clinic's call, pull up their care plan and ask brief follow-up questions Support the provider's preferences—such as helping request specialist or ER records, or checking in on medication adherence Assist with light documentation or communication as needed
Requirements:
Bilingual in English & Spanish (Required) Minimum 1 year of medical office experience Friendly, professional, and comfortable educating patients face-to-face Basic tech skills (smartphones, apps, and Bluetooth devices) Excellent communication and follow-up skills Familiarity with chronic conditions (diabetes, hypertension, etc.) EMR experience (Athena, Epic, or eClinicalWorks) is a plus Able to work independently while following clinic and provider workflows
