Documents and ensures that all clinical data, test results, the interpretation of the results by the doctor and healthcare providers are recorded accurately in the patients medical record file. Documents the correct time of patient care related activities, including practitioner to practitioner communication, family communication and reexamination of the patient. Educate and explain to patients about the risks and benefits involved. Procedures and treatments performed by all medical staff including doctors, nurses, medical assistants. Prepare referral letters as directed by the Doctor. Document requested medication refills, verify patient pharmacy, and send medications on the same day. Prepare medical letters, Home Health, FMLA, and disability paperwork as directed by the doctors as per summary of the medical record or as dictated by the physician. Also ensure the timely submission and send all letter with copies of patient reports or tests via mail, fax or email on daily basis to healthcare professionals involved in patients care. Research contact information for referring physicians, coordinate referrals, add referral information in EMR, make phone calls, and prepare operative reports. Identify and inform inconsistencies and errors / mistakes in documentation and review to correct the information in order to reduce errors, all changes / rectifications must be signed by doctor / physician. Notify doctors and healthcare professionals when documentation or chart is incomplete. Must comply with standards that apply to style of medical records and to the legal and ethical requirements for preparing documents and keeping patient information confidential as per state and federal laws. Comply with rules and guidelines as per HIPAA, OSHA and regulatory compliance, billing and coding. Assimilate new knowledge into processes and procedures. Proofread and edit all the physicians medical documents for accuracy, spelling, punctuation and grammar. Assists the practitioner in navigating the Electronic Health Record (EHR) and may locate information for review as requested by the practitioner (i.e., previous notes, reports, test results, and laboratory results). Also, may research information requested by the practitioner. Ensures the practitioner reviews all documentation completed by the Scribe when the patient encounter has concluded. The practitioner is relying on your accuracy. This includes making any necessary amendments and signing the chart. The practitioner is ultimately responsible for documentation of the patient's encounter. And any additional duties required by provider.
