What You'll Do:
Maintain and update reports and registries for the Patient Care Health Home (PCHH) team. Review registry data and ensure compliance with established workflows and procedures Complete audits for invasive procedures and in-house labs. Audit other PCHH data and initiatives as needed. Track all audits and provide documentation upon request Identify and collect process improvement ideas and communicate them to the appropriate staff Complete patient registration, verifying and updating insurance information Work with patients and staff to ensure completion of Release of Information (ROI) forms. Send subpoenas and court orders to the Health Information Management (HIM) department for processing Sort and distribute mail and faxes to the appropriate department or recipient Provide Obstetrics (OB) support as needed, including running reports, maintaining the OB log, and providing consent forms and records to the hospital Provide chart review support to Physician Assistants (PA) and Nurse Practitioners (NP) as needed, including forwarding chart reviews to physicians and maintaining a chart review log Manage, process, and track all referrals, ensuring all required forms are completed Manage various work queues daily to ensure timely processing and completion of referrals Respond to incoming calls, emails, and faxes regarding referrals, ensuring appropriate action is taken and documentation is completed and linked to the referral Set up appointments as applicable and coordinate follow-up between the referral source and the patient Assist with patient insurance authorizations, collaborating with internal resources as needed Perform other duties as assigned
Qualifications:
High School Diploma or General Education Diploma (GED) One year's experience working in an office setting, preferably a medical or dental office Bilingual (English/Spanish) speaking skills required at a level 9 Ability to prioritize work and handle a variety of tasks simultaneously, with frequent interruptions Ability to effectively interact with the public, providers and staff in a professional manner Basic proficiency with a variety of computer programs including Electronic Medical Records (EMR), Word and Excel Basic knowledge of medical terminology desired Our Mission Statement "Together we transform our communities through compassionate, individualized care, eliminating barriers to health and well-being."
