Medical Director / Attending Physician to oversee clinical care across partner facilities in Missouri. This role offers flexible delivery models, including a mix of in-person visits and telemedicine, fully supported by a dedicated Nurse Practitioner (NP) and administrative team. Clinic Model Base Stipend :
Fixed monthly stipend based on Part-Time or Full-Time status. Visit Revenue :
Receive a fixed rate of 60% of all collected insurance payments for both telemedicine and in-person visits. Cash-Pay Clients :
Receive a flat, guaranteed amount per client, paid out during the active pay period. 2. Long-Term Care (LTC) Division — Option 1 (Facility Percentage) Facility Stipend Share :
Earn 60% of the facility's monthly stipend (e.g., earn $1,200/month on a $2,000/month SNF contract). Note:
Facility stipends range from $150 to $2,000/month depending on facility type and budget; some facilities do not offer a stipend. Medical Director Visits :
Earn 55% of collected insurance claims for in-person and telemedicine visits. Attending Physician Visits :
Earn 60% of collected insurance claims (higher percentage balanced by the exclusion of facility or collaboration stipends). 3. Long-Term Care (LTC) Division — Option 2 (Collaboration Stipend) Flat Collaboration Fee :
A steady, monthly flat fee paid per assigned Advanced Practice Provider (NP). Tiered Flex Structure :
Scale your earnings as the practice grows (e.g., localized flat rates that dynamically adjust based on your specific ratio of buildings to collaborating NPs). Medical Director Visits :
Earn 55% of collected insurance claims for all visits. Attending Physician Visits :

100% company-paid and maintained professional liability coverage ($1,000,000 per claim / $3,000,000 aggregate). Continuing Education (CME) : Annual allowance up to $3,500 plus 5 paid days off (compensated at $50/hour, 8-hour days), scaled to your collaboration volume and facility count. Licensure & Credentialing :

Position Overview You will serve as either an Attending Physician or Medical Director (based on facility assignment) across Residential Care Facilities (RCF), Assisted Living Facilities (ALF), and Skilled Nursing/Long-Term Care Facilities (SNF/LTC). An optional outpatient clinic component is also available. Key Responsibilities Residential & Assisted Living Care (RCF/ALF) Annual Visits :
Ensure each assigned resident receives at least one clinical visit per year (delivered via telemedicine or in-person). Collaborative Oversight :
Review and sign state-restricted clinical documentation that Missouri law prohibits APRNs from signing independently. Prescription Control :
Manage electronic prescribing and review of controlled substances that exceed Missouri APRN statutory limits. Skilled Nursing & Long-Term Care (SNF/LTC) On-Site Visits :
Conduct physical, on-site visits to assigned SNF/LTC facilities every two weeks. Patient Prioritization :
Focus visits on Medicare Part A and Managed Care Skilled patients first, followed by long-term care residents. Practice Support & WorkflowWe provide a highly structured support system to minimize your administrative burden: Clinical Support :
Dedicated NPs (Medical and Psych) handle routine rounds based on facility needs (ranging from twice weekly to bi-weekly). Administrative Relief :
Support staff and NPs manage the majority of background tasks, including prior authorizations, therapy signatures, compliance paperwork, care plan updates, medication reconciliation, and note preparation. Streamlined Logistics :

A full-time, 24/7 provider is on staff to handle all routine medical and psychiatric on-call duties. Qualifications Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree. Active, unrestricted medical license in the State of Missouri. Board certification or eligibility in Family Medicine, Internal Medicine, Geriatrics, or a related specialty. Active DEA registration and Missouri BNDD registration. Pay:
