Primarily daytime hours, with flexibility for some remote documentation Position Overview Rehabilitation Associates is a leading multispecialty medical group focused on advancing post-acute and long-term care through an interdisciplinary, team-based approach. We are seeking a dedicated and experienced Medical Director, MD or DO , to provide clinical leadership and direct patient care across nursing and residential care facilities in the greater Bangor area. The Medical Director will work closely with an established team of physicians, Advanced Practice Providers, facility leaders, nursing staff, and other members of the interdisciplinary care team. This position combines clinical practice with medical director responsibilities. The physician will support the management of acute medical issues affecting short-term rehabilitation patients, chronic and complex conditions among long-term care residents, and the quality, safety, and regulatory priorities of the facilities served. The position is structured to promote professional fulfillment and work-life balance through a collaborative practice model, strong administrative and clinical support, and flexibility for some documentation to be completed remotely. Primary Responsibilities Direct Patient Care Provide comprehensive medical care to patients in subacute rehabilitation, skilled nursing, long-term care, and residential care settings. Perform initial evaluations, admission visits, regulatory visits, acute visits, and clinically appropriate follow-up visits. Obtain and review medical histories, perform examinations, and assess patients' medical and functional status. Diagnose and manage acute, chronic, and medically complex conditions. Order and interpret diagnostic testing, laboratory studies, imaging, and other clinically appropriate services. Prescribe medications and develop, implement, and revise individualized treatment plans. Coordinate specialty consultations, referrals, hospital transfers, and other transitions of care when clinically indicated. Support timely and clinically appropriate admission, transfer, and discharge planning. Counsel patients and families regarding diagnoses, prognosis, treatment options, preventive care, goals of care, and advance care planning. Maintain complete, accurate, and timely medical documentation in accordance with organizational, payer, and regulatory requirements. Medical Direction and Clinical Leadership Provide medical leadership and oversight consistent with applicable federal and state regulations, professional standards, and facility agreements. Collaborate with facility administration and clinical leadership to establish and maintain appropriate standards of medical care. Support the development, implementation, and review of clinical policies, procedures, protocols, and care pathways. Provide clinical guidance and consultation to Advanced Practice Providers and other members of the care team. Promote effective communication and collaboration among physicians, APPs, nursing staff, rehabilitation professionals, pharmacy, social services, behavioral health, and other disciplines. Assist with the review of complex cases, significant changes in condition, adverse events, hospital transfers, and other high-risk clinical situations. Support practitioner performance, clinical consistency, and adherence to accepted standards of care. Participate in provider onboarding, education, mentoring, and professional development as appropriate. Serve as a clinical liaison between Rehabilitation Associates and facility leadership. Quality, Safety, and Regulatory Oversight Participate in Quality Assurance and Performance Improvement meetings and other required facility committees. Review relevant facility clinical and quality metrics, including potentially avoidable hospitalizations, emergency department utilization, readmissions, medication utilization, infection trends, and other patient outcome measures. Participate in the development and implementation of performance improvement initiatives. Support medication management and pharmacy-related quality efforts, including psychotropic medication stewardship, antibiotic stewardship, polypharmacy review, and appropriate documentation of medication indications. Contribute to infection prevention, patient safety, fall prevention, skin integrity, behavioral health, dementia care, and other facility quality initiatives. Assist facilities with clinical preparation for regulatory surveys and the development of corrective action plans when appropriate. Participate in the review of facility assessments, clinical policies, care practices, and regulatory compliance concerns. Support the facility's efforts to ensure timely practitioner visits, appropriate clinical follow-up, and continuity of care. Provide medical input regarding significant clinical events, grievances, risk-management concerns, and quality-of-care reviews. Interdisciplinary and Facility Collaboration Participate in interdisciplinary care conferences, clinical meetings, and facility leadership discussions as appropriate. Collaborate with the care team to identify barriers to successful rehabilitation, safe transitions, chronic disease management, and resident quality of life. Promote patient-centered care that reflects individual goals, preferences, functional needs, and clinical circumstances. Maintain professional and responsive communication with facility staff, patients, families, and external healthcare partners. Foster a culture of accountability, collaboration, clinical excellence, and continuous improvement. Qualifications MD or DO with a valid Maine medical license. Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or a related field. Prior experience in subacute or long-term care settings is preferred. Strong communication and interpersonal skills. Commitment to providing high-quality patient care. Ability to work effectively in a collaborative, team-based environment.
