General Description:
Corazon Ministries seeks a compassionate and mission-driven Nurse Practitioner to lead and expand healthcare services for individuals experiencing homelessness. This position provides direct medical care to unsheltered and vulnerable populations through onsite services at a day center and resource hub, as well as through street medicine outreach. The Nurse Practitioner will supervise clinical staff, oversee healthcare operations, and partner with executive leadership to advance the mission of Corazon Ministries to include building and running a medical respite / hospice program for the unsheltered in San Antonio.
Essential Functions:
Provide primary, preventative, and urgent healthcare services to unsheltered individuals and other program participants using evidence-based, trauma-informed, culturally responsive, and patient-centered approaches. Provide healthcare in nontraditional environments including encampments, shelters, transitional housing, and community outreach settings. Conduct comprehensive health assessments, screenings, and evaluations including coordinating laboratory testing, immunization referrals, and STD/STI testing, treatment, education, and follow-up care. Prescribe, monitor, and manage medications in accordance with applicable laws and clinical standards. Perform wound care, chronic disease management, and basic urgent care services. Develop individualized care plans and coordinate referrals to hospitals, specialists, behavioral health providers, and community partners. Maintain accurate and compliant medical documentation and records. Provide clinical leadership, supervision, mentoring, and performance oversight for nurses, medical assistants, outreach clinicians, volunteers, and interdisciplinary healthcare team members. Oversee clinical operations at the day center/resource hub and street medicine program. Develop and implement evidence-based clinical policies, protocols, and standard operating procedures. Lead quality improvement initiatives and participate in program evaluation using clinical outcome measures. Collaborate with executive leadership, physicians, PA's, nurses, social worker, behavioral health, EMS, hospitals, public health, and housing agencies to develop healthcare strategies, partnerships, and program growth initiatives. Represent Corazon Ministries with healthcare partners, funders, and community stakeholders.
Qualifications:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program. Current Nurse Practitioner license in good standing. Current DEA registration and Texas prescriptive authority or ability to obtain prior to employment. Minimum of two (2) years of clinical experience; three to five years of primary care or community health experience preferred. Experience working with vulnerable, homeless, underserved, or medically complex populations, particularly in street medicine, homeless healthcare, community health, addiction medicine, behavioral health, wound care, or harm reduction, preferred. Leadership or supervisory experience is preferred. Experience with electronic health records (EHR) preferred. Current BLS certification required; additional certifications are a plus. Strong communication, organizational, and interdisciplinary collaboration skills. Valid Texas driver's license and reliable transportation Commitment to serving others and advancing the mission and values of Corazon Ministries. Work Environment General hours of operation are Monday through Friday, 8:00 a.m. to 5:00 p.m. Hours may vary based on program needs, outreach activities, emergencies, and community events. Work is performed in office, clinic, community, and outdoor street outreach settings. May involve standing, walking, lifting supplies, and traveling throughout the service area.
Pay:
$120,000.00 per year
