Job Description

We are hiring a TemporaryTime Medical Records Clerk at Antelope Valley Cardiology located in Lancaster, CA.
Summary:
This position creates new medical records and retrieves existing medical records by gathering appropriate record folders and contents;; verifying existing record numbers; inputting and recording locations to computer; delivering records.
Essential Responsibilities and Duties:
Gathers patient information by collecting demographic information from a variety of sources; retrieving information from e-fax Preload patient health information into Electronic Medical Record (EMR) for upcoming appointment from referring physician records or from patient's paper chart. Scan paper documents into EMR system and import documents into the correct patient EMR. Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes. Keeps health care providers informed by communicating availability or unavailability of the record. Maintains quality results by following ASC policies and procedures. Maintains patient confidence by keeping patient records information confidential. Serves and protects the healthcare community by adhering to professional standards, ASC policies and procedures, federal, state, and local requirements, and AAAHC standards. Enhances medical records and hospital reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments. Take request for patient records from outside physician offices within HIPPA compliance. Other duties as assigned.
Qualifications:
High school diploma or equivalent is required. Prior experience with medical records preferred. Must have excellent computer and communication skills. Accurate and speedy data entry skills.
