Job Description
Basic Function Responsible for making sure every patient seen within the practice has an organized medical record that is legible, accurate, and readily accessible, and easily retrievable. They must keep medical records in a secure place to assure limited access. Responsibilities including but not limited to: Make sure every patient seen has a medical record 2. Ensure that all necessary forms are in the record. 3. Files and stores records in alpha order by the patient's last name. 4. When a record is removed, a record locator will be inserted in its place. 5. Will file most recent documents on top of previous documents of the same type. 6. Will ensure patient name is on every document. 7. Will release medical record only with appropriate authorization. 8. Will maintain the confidentiality of patient information and medical records. 9. Will purge medical records if the patient has not been seen in three years. 10. Will comply with State and Federal Laws regarding subpoenas and court orders. 11. Will periodically perform chart review.
