Summary:

The Health Information Release Specialist receives requests for health information distribution and releases as authorized and appropriate.

Responsibilities:
  • Serves as the first point of contact for the department. Receives medical requests over a range of mediums including phone, in-person, fax and email.
  • Confirm the identify of requestors using government-issued methods of identification. Reviews written authorization from patients prior to release.
  • Ensures that a HIPAA-compliant Authorization for the Release of Medical Information is obtained prior to the release of records outside the organization, and in strict adherence to HIPAA and hospital patient privacy guidelines as defined in the Release of Information procedure.
Authorizations should include:

patient name, signature of patient or legal representative, name of the requester, dates of service requested are prior to the date signed, and within policy-defined time period constraints, and that relevant Alcohol/Substance Abuse/Psychiatric/or HIV diagnoses are included in the body of the authorization.

  • Collaborates with Pardee legal counsel regarding subpoenas and court orders for medical information or when legal issues arise regarding disclosure of health information to a third party.
  • Documents and direct patient complaints or grievances with organizational risk management team.
  • Receives and responds to governmental information requests requiring special and/or expedited attention such as concealed weapon permits or Social Services requests.
  • Responsible for completing and notarizing signatures on legal documents to include affidavits, power of attorneys, advance directives, and other legal documents as part of notarization duties.
  • Documents all releases into electronic medical recordkeeping (EMR) tool.

QUALIFICATIONS

Hired applicants will be required to obtain notary public commission. Training opportunities will be provided at no cost.

Required

  • High school degree or equivalent
  • Familiarity with desktop computing tools and software
    Preferred
  • Current Notary Public commission
  • Associate's degree
  • Training or education in Medical Terminology
