Job Description

Job Overview We are seeking a detail-oriented and highly organized Medical Records Clerk to join our healthcare team. In this vital role, you will manage and maintain accurate medical records, ensuring compliance with healthcare regulations.
Scope of Position:
To ensure that patient files are complete and up-to-date at all times.
Qualifications:
Minimal Qualifications:
1. High school graduate or GED. 2. Training and/or experience in general office procedures. 3. Training and/or experience in operation of standard office equipment. 4. Excellent verbal and written communication skills required. 5. Knowledge of medical terminology is preferred. 6. Ability to type 50 words per minute preferred. 7. Computer experience is preferred. 8. Attendance requirements 8:00 to 5:00 generally, Monday through Friday. Hours may vary.
Responsibilities/duties:
1. Assists clerical staff as needed. 2. Assists in ensuring smooth operation of the office. 3. Files all elements of patient/client records on a daily basis. 4. Typing. 5. Acts as backup receptionist as needed. 6. Complies with agency policies and procedures. 7. Maintains employee and patient confidentiality. 8. Restricts access to patient files to authorized personnel. 9. Maintains control log over location of patient charts by signing each chart in and out of file room. 10. Assembles all active charts after all admission paperwork has been completed. 11. Maintains current log of interim orders and plans of care (POCs) to verify mail and return date to comply with state and federal regulations. 12. Audits charts using chart checklist to ensure all required information is in chart; informs nurse or home health aide of any discrepancies. 13. Initiates medical charts for new admits as directed. 14. Handles requests for information, which can be located in patient charts.
