Job Summary:

Assumes responsibility for collecting and verifying the accuracy of birth certificate worksheet data and entering and transmitting this data to local State and County registrar within the prescribed time-frame. Reviews patient records and codes birth certificates accordingly. Collects and reviews all paternity forms. Submits paternity forms to State and County registrars. Works with sensitive and confidential information.

Education and Work Experience:

High School Education/GED or equivalent: Preferred Associate's/Technical Degree or equivalent combination of education/related experience: Preferred One year's relevant experience:

Basic Life Support (BLS) certification from approved vendor per AH policy:

Required Essential Functions:

Collects and ensures all documentation on birth certificate worksheets are complete and accurate. Reviews medical records and codes each birth certificate accordingly. Enters all initial data and amendments for birth certificates, transmits to Health Department within the allocated time frame and submits all worksheets to HIM. Collects, reviews and sends Paternity documentation forms from patients to State and County monthly. Performs other job-related duties as assigned.
