Job Description

POSITION SUMMARY
The Referral Support Clerk assists with chart preparation, quality care gap identification, and chart audits to ensure comprehensive patient records. They support referral and VA authorization processes. The role requires strong organizational skills, attention to detail, and the ability to collaborate effectively with healthcare providers and patients.
DUTIES & RESPONSIBILITIES
Ensure patient charts are complete, accurate, and up to date. Maintains medical chart security and HIPAA compliance. Provides organized information to providers upon immediate request. Completes Quality Assurance duties to include chart audits, gap closures, letters and scanning as required. Support referral and VA authorization processes. Uses tact in dealing with patients, physicians, and other professional inquiries. Obtains and delivers medical information to nursing staff prior to patient appointments as requested. Researches and delivers various types of information sought by physicians for continuity of care. Provides office support to all physicians as needed. Demonstrates appropriateness in meeting objectives in age-specifics. Performs other duties as assigned.
QUALIFICATIONS
High school graduate or equivalent. Previous clinic or medical office experience preferred. Exceptional organizational skills. Familiarity with office equipment (fax machine, copier, etc.) Ability to establish and maintain working relationships with patients, providers and other employees. Professional telephone skills. Ability to work independently and as a team member. CPR Certified (obtain upon employment)
