All Shifts Job Summary:
Senior Living Experience Preferred Job Duties:
Assist residents with administration of medications per physician orders — reminding residents, retrieving medications from centralized storage, and handing them to the resident at scheduled times. Accurately complete and maintain each resident's Medication Administration Record (MAR), including dosage, time, and any refusals, omissions, or errors. Observe residents for side effects, adverse reactions, or changes in condition and report findings promptly to the Resident Care Director and appropriate medical personnel. Maintain proper storage, labeling, security, and inventory of centrally stored medications per Title 22, § 87465. Coordinate medication refills, reorders, and returns with the contracted pharmacy; monitor expiration dates and ensure proper disposal of discontinued or expired medications. Communicate with residents' physicians, pharmacists, and responsible parties regarding medication-related questions, as directed. Document and report medication errors or incidents per facility policy. Participate in care plan reviews and communicate medication-related observations during care conferences. Maintain accurate records in the facility's EHR/EMR system (e.g., AL Advantage), as applicable. Maintain resident confidentiality consistent with facility policy and applicable law. Perform other duties as assigned by the Resident Care Director or Administrator. Provide assistance or reminders with activities of daily living, including grooming, oral hygiene, bath/shower, hair cleaning and brushing, and transferring to and from activities and meals, as needed. Provide assistance with resident's personal environment (e.g. clothing, linen, and personal belongings). Lift and/or transfer residents as needed. Record and report changes in resident's condition to the supervisor including but not limited to changes in resident's ability to perform activities of daily living, skin changes, bruising, etc. Immediately report all incidents or accident involving residents to the Resident Care Director/Supervisor and record all necessary information on the Resident Incident and Accident Reporting Form. Follow proper procedures in emergency situations and respond promptly and positively to resident requests for assistance, including emergency pull cords, telephone calls, and requests from family and friends. Correct and prevent environmental hazards in an appropriate and safe manner while observing universal precautions and infection control procedures. Immediately report any suspected abuse, neglect or misappropriation of resident funds or property to immediate supervisor. Perform work assignments and attend required in-service training.
