Department:
Outpatient Pharmacy The Pharmacy Medication Reconciliation Technician is under the direct supervision of the Director of Pharmacy or his/her designee, with support from the Pharmacy Transitions of Care Coordinator. In accordance with department policies and procedures and guidelines, the Pharmacy Medication Reconciliation Technician will assist pharmacists in obtaining a list of the most accurate prior to admission medications by interviewing patients and consulting with their outpatient pharmacies. The Medication Reconciliation Technician will obtain medication histories in the Emergency Department as well as in the inpatient areas. Through performing medication histories, the Medication Reconciliation Technician plays an integral role in helping to prevent medication errors caused by inaccurate or incomplete information. The Pharmacy Medication Reconciliation Technician will be expected to provide exemplary service to other internal customers at SVMHS, as well as embody SVMHS's STAR values in all interactions with patients and colleagues. Collaborates with unit care providers to determine when patients can be interviewed and obtains medication histories on all assigned patients. Establish rapport with patient to obtain medication history or determine if patient or family member can provide medication history. Has the ability to navigate an electronic Health data base and enter new medication history or if available update existing medication profile. Identifies medication names, common dosages, dosage forms and frequency of administration for all medications. Assess incompleteness of current list of medications looking at drug, dose, frequency, indication for use. Determines time last dose of each medication was taken prior to admission for future scheduling. Determines compliance of medication consumption prior to admission. Obtains indication for PRN medications and actual frequency of use of PRN medication. Prompts patient for additional medications, by specifically suggesting over the counter (OTCs) medications including herbals, vitamins, pain relievers, cold products patches, lotions & creams, eye drops, sprays, and inhalers. Calls outpatient pharmacies or other healthcare facilities to confirm medication regimens or investigate discrepancies when necessary. Determine if patient was on recent antibiotics prior to admission. Verifies accuracy of taken medication history by source confirmation. Obtain medication allergy history and related reactions to the medication. Maintains the confidentiality of all designated hospital and patient information and complies with HIPPA standards at all times. Conducts interpersonal relationships with patients, visitors, physicians and fellow employees in a courteous and customer friendly manner. Follows established hospital infection control and safety procedures. Performs other duties as assigned.
Education:
A minimum of a High School Diploma required. Successful completion of an accredited Pharmacy Technician certification course required.
Licensure:
Current California Pharmacy Technician certification required.
Experience:
A minimum of four (4) years' experience in retail pharmacy required. Excellent knowledge of brand and generic medications required. Has ability to communicate with health care team members and patients of all education levels. The hourly rate for this position is $34.36 - $37.89. The range displayed on this job posting reflects the target for new hire salaries for this position.
