3 pm to 11 pm Weekends Availability neededDescription of Caregiver Position:
Provides direct personal care and supervision to the residents at the Assisted Living Facility. Promotes resident well-being and satisfaction through support with activities of daily living. Communicates with other departments to ensure resident needs are met. Each Caregiver will be charged with all of the personal care duties of their resident assignment. Whenever possible each Caregiver will be assigned to the same resident group each day to promote continuity of care. Responsibilities of theCaregiver:
Assists with activities of daily living following Assisted Living Facility protocol, licensing regulations, and guidelines for both resident and employee safety. Follows safety guidelines in the Assisted Living Facility, including proper lifting technique and universal precautions, when providing care to the residents. Follows the schedule of duties for the Caregiver, as well as the individual plan of care for each resident. Functions as a team, assisting co-workers as the need arises throughout the work shift. Monitors residents activities, food intake, functional status, psycho-social status, and takes action as required to promote residents well-being. Reports residents behaviors changes immediately to the supervisor. Acts immediately on any residents crisis, following established protocol. Documents residents status changes, including but not limited to, physical changes, reaction to medication, psycho-social status changes, and behavioral changes. In the event all assigned duties cannot be completed, the Caregiver asks for assistance and reports to the Resident Care Coordinator or Administrator. Promotes open communication between health care professionals, families, residents, and staff. Adheres to guidelines in the employee handbook including dress code, conduct, scheduling, etc. _____________________________________________________________Med-Tech Description of Position:
Coordinates residents care related to assistance with medications, by working with all departments (the assisted living facility, families and administrative staff) to provide for resident needs with continuity and in adherence to the scope of practice and license requirements for the Assisted Living Facility. Provides complete supervision of the medication room, pass techniques, documentation and supervision and provision of care related to medication in the Assisted Living Facility. The Assisted Living Facility has at least one Medication Aide on each shift. Responsibilities of theMedicine Aide:
Residents charts: Keeps documentation current (Assisted Living Facility forms, physician orders, incident reports, etc.). Communicates residents status changes. Ensures all medication documentation is current and correct, including medication administration forms, physician orders, change of dosages, written orders to confirm telephone orders, etc. Ensures medication room is completely stocked with all required continuous, PRN, Over-the-Counter (OTC), and other medications as ordered by the physician. Coordinates medication orders and deliveries with pharmacies, if applicable. Communicates with physicians and other healthcare providers as needed and upon request of Nurse/Administrator. Monitors residents behaviors and reports any changes in residents behaviors to physician and Administrator/Nurse. Controls medication access. Pours, passes, and assists with administration of medications in accordance with state regulations. Coordinates physician and other medical appointments, as needed. Reads all communication notes regarding the Assisted Living Facility between the Caregiver shifts. Other duties as assigned, such as Caregiver duties, etc. Adheres to guidelines in the employee handbook including dress code, conduct, scheduling, etc. ______________________________________Brand:
