Responsibilities Assist residents and patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment when necessary. Administer prescribed medications accurately following all safety protocols and document each administration in electronic health record (EHR) systems like . (Mar sheets) Monitor vital signs, observe changes in physical or mental condition, and report findings promptly to nursing staff or supervisors. Support specialized care needs including Alzheimer's care, memory care, behavioral health management, and infection control procedures aligned with aseptic techniques. Maintain detailed medical documentation, ensure compliance with HIPAA regulations, and update medical records consistently. Collaborate with interdisciplinary teams in settings such as long-term care facilities, assisted living communities, hospitals—including ICU or trauma centers—and outpatient clinics. Use medical terminology confidently to communicate patient status clearly and effectively across healthcare teams. Requirements Valid certification as a LPN, Certified Nursing Assistant (CNA) and medication technician certification must be valid. Experience working in diverse healthcare environments such as hospitals (including ICU or emergency medicine), nursing homes, assisted living facilities, or home & community care settings. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate documentation. , and basic life support (BLS) procedures. Experience administering medications safely and accurately in accordance with state regulations and facility policies. Ability to work effectively with individuals across the lifespan—including pediatrics and geriatrics—and those with disabilities or behavioral health needs.
