Obtains and documents an accurate medication list for patients newly admitted to the hospital to help avoid medication duplications, omissions, and errors. In an accurate, professional and efficient manner, interviews patient for information and when necessary, utilizes drug information resources and contacts community pharmacies, outpatient providers, and patient family members to obtain needed medication information and coordinate care.

Responsibilities:

Carefully and diligently collect medication histories from various sources Accurately enter information into multiple EMRs and pharmacy informatics systems to be used by physicians and other health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital Communicate professionally with other members of the health care team Familiar with standard concepts, practices, and procedures within the field Relies on experience and judgment to plan and accomplish goals Regular and predictable attendance is an essential job function Competent to meet age specific needs of the unit assigned

Requirements and Minimum Qualifications:

Successful completion of a licensed Pharmacy Technician Program or 2 years' experience in a retail pharmacy, pharmaceutical company or equivalent work setting Idaho State Board Pharmacy Technician registration required National certification through PTCB or ICPT required Six months of Pharmacy Tech II experience at Kootenai Health or 12 months of equivalent pharmacy experience required Demonstrated ability to use computer skills including keyboarding and practical data entry into data based software programs Effective patient interviewing skills Detail oriented, ability to handle multiple tasks simultaneously, and the ability to meet deadlines and work with frequent interruptions Knowledge of Laws or regulations regarding Privacy Act and Confidentiality required
