Job Description

LIBERTY HEALTH CARE CENTER
- Liberty, TX Skilled Nursing & Long-Term Care | Full-time Days (7am-7pm) Serve with heart. Support residents with safe, reliable care. Liberty Health Care Center is seeking a compassionate, detail-oriented Certified Medication Aide to support our residents and nursing team. Under the direction of an RN or LVN, you'll administer medications safely, assist with daily care, and help maintain a clean, comfortable environment. What You'll Do Administer medications per provider orders and scope of practice; follow the Five Rights and document accurately in the MAR/TAR. Take and record vital signs, observe residents, and report changes promptly to the nurse.
Maintain medication carts/rooms:
stocking, expiration checks, counts (including controlled substances), and sanitation. Assist residents with ADLs (bathing, dressing, grooming, hygiene) and ensure comfort and dignity. Follow infection control and safety procedures; support fall prevention and elopement protocols. Support admissions/returns (orientation, basic education, belongings logs) and coordinate nourishments as directed. Communicate respectfully with residents/families; protect
PHI/HIPAA
at all times. Complete required documentation, incident reports, and in-service trainings. Other duties as assigned to support resident care and the nursing unit. Qualifications Current Texas Medication Aide permit (unencumbered) required. 1+ year recent CMA experience in SNF/LTC preferred. Current CPR/BLS (or willingness to obtain promptly). Strong communication, teamwork, and customer-service skills; compassionate, patient demeanor. Ability to follow care plans, med schedules, and written/oral directions. Basic EMR familiarity (PointClickCare a plus). Able to stand/walk most of the shift and safely assist with resident care; lift/transfer within policy. Meets all pre-employment/health requirements (background, drug screen, TB/clearances).
