#CareHawaiiLP •Open positions are located in the following areas, Oahu (Pearl City and Kaneohe), Hawaii (Kona). We are full and part time positions open, looking to fill nights and weekends immediately!
Position Summary:
Care Hawaii is a behavioral health care organization seeking a compassionate and dependable Case Manger to support the daily functions of CAREs provision of services. CARE Hawaii, Inc. contracts with multiple stakeholders to provide an array of services. The primary duty of the Mental Health Worker is to provide crisis stabilization and crisis support management to individuals experiencing immediate and often life-threatening crisis situations in a residential setting. The Mental Health Worker has advanced training in a mental health profession (i.e., nursing, social work, psychology) and clinical skills.Position Qualifications:
Minimum Education:
Bachelor's degree from a nationally accredited college or university in the Humans Services field, or close relatedMinimum Experience:
1 Year of experience in Human Services or closely related fieldRequired Certification/Registration:
CPR and First Aid TB Background Check CPI Valid Driver's License Vehicle:
Current and Valid Documentation Duties and Responsibilities:
Provide crisis stabilization and crisis support management based on risk assessment Function independently as well as in collaboration with other members of the crisis team as needed Assume primary responsibility for developing, writing, implementing, evaluating, and revising crisis plans such as the Wellness Recovery Action Plan (WRAP) and Crisis Triage Rating Scale (CTRS) in conjunction with the other members of the crisis team Provide individual crisis stabilization and crisis management, ensuring immediate changes are made in the crisis plans as clients' needs change Educate and support clients' families, and advocate for clients' rights and preferences Conduct comprehensive assessment of psychiatric history, mental status, and diagnosis; physical/dental health; use of drugs or alcohol, education/employment; social development and functioning; activities of daily living; and family structure and relationships. Submit charts within seven (7) days. Document client progress to maintain a permanent record of client activity according to established methods and procedures Provide ongoing assessment and response to crisis management Provide transportation when necessary to clients Provide symptom education to enable clients to identify their mental illness symptoms Provide direct clinical "in-vivo" services on an individual, group, and family basis Provide vocational-supportive counselingJob Type:
