Qualifications:
Qualified applicants should have knowledge of mental health diagnosis such as anxiety, depression, bipolar, and other related disorders. Qualified applicants need to have and/or have a willingness to be trained on how to write medical and service documentation. Qualified applicants need to have strong organizational skills. Qualified applicants should have strong communication skills. Qualified applicants must have a valid drivers license, a car, and car insurance. Qualified applicants must be able to work independently and create a schedule according to the assigned clients they will be working with. Preferred, but not required: Applicants have the knowledge of local resources in the community where applying. Qualified applicants must be able to pass Criminal and DHS background checks.
Job Duties:
The Behavioral Health Consultant is supervised by the Executive Director, and is responsible for providing supportive community living services to the Clients of Symmetry Health. Responsibilities shall include, but are not limited to: To meet with Clients in their most natural setting (home or community). To assist Clients in developing natural supports. To assist Clients in becoming as independent as possible. To contact Clients if appointments need to be changed or if counselor is home ill. To treat Clients and other staff with dignity and respect. To attend all Symmetry Health staffing and training sessions as requested by the Executive Director. To follow all Symmetry Health policies and procedures; including Ethics, Mandatory Reporting, and Rules of Confidentiality. To obtain the proper diagnosis information on assigned Clients according to Symmetry Health policies and procedures. To review and update diagnosis information and Client file contents according to Symmetry Health policies and procedures. To complete quarterly progress reports on assigned primary Clients. To meet for designated meetings with the Symmetry Health team. To work Monday through Friday 9:00am through 5:00pm, unless approved by the Director of Program Operations. To be mindful of time management and arrive to work or scheduled appointments and responsibilities in a prompt manner. To maintain assigned Client file contents and requirements outlined by Symmetry Health policies and procedures and according to Chapter 24 and HCBS guidelines. To complete service documentation within 48 hours of the time of real contact on assigned Clients. To perform and complete annual updates on all assigned HCBS and Chapter 24 Client reviews (including social history, assessment, Individual Service Plan packet, Crisis Intervention Plan, and all other annual updates). To develop an Individual Service Plan on assigned Clients and meet with case managers. To check messages and respond in a timely manner (within 24 hours) to all business email, phone calls, and text messages. To submit properly completed payroll forms, mileage sheets, and service documentation each week.
