Job Description

Job Summary Join our dedicated team as a Community Networking/Day Supports professional, where you will play a vital role in empowering individuals with mental disabilities lead fulfilling, independent lives. This energetic position involves building strong community connections, providing personalized day supports, and assisting clients with daily activities. Your enthusiasm and compassion will foster a positive environment that promotes growth, social engagement, and community integration for those we serve. This paid position offers an exciting opportunity to make a meaningful difference while developing your skills in behavioral health, special education, and caregiving. Duties Develop and implement individualized support plans that promote community participation and social skills. Assist clients with activities of daily living (ADLs), including grooming, hygiene, meal preparation, and transportation. Facilitate engaging community outings such as recreational activities, appointments, and social events to enhance independence and socialization. Utilize effective behavior management techniques to support positive behavior and address challenging behaviors. Collect and document data related to client progress, service delivery, and behavioral observations accurately and consistently. Administer medications following prescribed protocols when necessary and ensure compliance with safety standards. Maintain a safe environment by adhering to health and safety regulations during all activities. Collaborate with families, caregivers, behavioral health professionals, and educators to ensure comprehensive support for each individual. Drive clients safely to appointments or community activities using company vehicles while maintaining proper driving records. Qualifications Experience working with individuals with developmental disabilities, autism, or behavioral health needs preferred. Knowledge of special education principles and behavior management strategies. Strong communication skills with the ability to connect effectively with diverse populations. Home & community care experience is highly desirable. Ability to assist with ADLs and provide compassionate caregiving in various settings. Familiarity with data collection methods related to client progress and behavioral tracking. Valid driver's license with a clean driving record for transportation duties.
