POSITION SUMMARY
The WellLife Network Children's The CSPOA Mental Health Professional serves an essential role within Suffolk County's Children's Single Point of Access, a centralized referral, assessment, and monitoring system for youth and young adults ages 5 to 21 who present with serious emotional disturbance or significant behavioral health needs. This position is responsible for reviewing and evaluating applications, identifying appropriate service pathways, supporting linkage to community-based resources, and coordinating with families, providers, schools, and system partners. The role helps ensure that youth receive timely access to the least restrictive and most appropriate level of care, with the goal of supporting stability, strengthening family functioning, preventing unnecessary out of home placement, and helping youth remain safely connected to their homes, schools, and communities.
ESSENTIAL FUNCTIONS
Accurately enter applications, transfers and inactivations, notes and other relevant documents in the CSPOA database/EHR; Assist in monitoring referral status, including incomplete referrals and follow-up with referents and other individuals to gather additional information as necessary; Handle inquiries requesting information about CSPOA programs and services; Make presentations at various outside agencies on an as needed basis; Maintain files, including creating new files, tracking closed files, scanning and archiving closed files for storage, and maintaining a well-organized file room; File paper forms in client files; scan and upload documents into the EHR; Utilize the CSPOA Access database/EHR and general IT applications (Ex/ Word, Excel, Adobe) in a proficient manner; Understands and demonstrates commitment to agency risk management procedures and adheres to HIPAA regulations; Perform all other job duties as assigned.

Efficient time management and ability to meet deadlines. Proficiency in standard IT applications (Word, Excel, Adobe, etc.). Qualifications Master's degree in social work or related field, limited permits will be considered, at least one year of experience in behavioral health or human services working with youth, adults and/or families. The position calls for strong communication and collaboration with families, healthcare providers, and community partners, as well as flexibility, professionalism, and adherence to required training.
