Job Description

Position 1 : Friday and Saturday 11p-7a
Position Definition:
Provide Consumers with support and skills training to promote wellness and independence.
Job Responsibilities:
Assist consumers with the development and implementation of individual goal plans or individual residential service plans; monitor consumer progress in goal areas; maintain and update documentation in consumers individual records; assure consumer safety and wellbeing; respond immediately to emergency situations affecting consumers' health and safety; report any signs of consumer decompensation to supervisor; assist consumers with menu planning, grocery shopping, meal preparation, daily/weekly housekeeping tasks; provide limited consumer transportation; follow and abide by confidentiality policies; perform related duties a assigned.
Qualifications:
High school diploma or equivalent; valid pa driver's license; and at least 1 year experience in the mental health field.
Skill:
Ability to manage a crisis/emergency situations; self-motivated; good oral and written communications skills.
