Job Description

Mental Health Liaison Nurocoach Job Summary:
The Mental Health Liaison is responsible for the coordination of patient care activities between Nurocoach (an integrated psychiatry practice) and our referral practices. This specific role is onsite at a pain management clinic in north Dallas, starting at 3 days per week with the potential for full-time employment within 2-3 months. We are looking for a candidate with a STRONG focus on customer service and ongoing process improvement. The Mental Health Liaison is responsible for identifying chronic pain patients who will benefit from mental health support, and then coordinating with onsite management and personnel to position Nurocoach as a resource to optimize their treatment outcomes. Nurocoach has a strong record of promoting from within as the company grows. This role and engagement are a pilot program, for which leadership positions will immediately be available for the right candidates.
Essential Job Functions:
Coordinate referrals with referral client personnel, including direct coordination with patients onsite Facilitate new patient intakes for mental health support Direct patient coordination for various ancillary programs related to mental health and chronic care management Drive patient volume and strategic referral partnership with referral client management and office personnel
Requirements:
Bachelors degree preferred; or 5+ years working in a clinical environment Must be technically proficient with electronic medical records (EMR) software. This role will require proficiency to manage TWO EMRs at once (eClinicalWorks & AdvancedMD), for which we will provide training Must possess a valid driver's license and be able to commute to client office sites in DFW metro Compensation based on experience and education level.
