You will work closely with clients to promote stability, independence, and well-being through tailored interventions and comprehensive care. Responsibilities Provide direct mental health services and support within clients' homes, ensuring a safe and nurturing environment. Conduct assessments to identify individual needs, develop personalized care plans, and implement therapeutic strategies aligned with best practices. Manage crisis situations effectively through crisis intervention techniques and crisis management protocols to ensure client safety. Support clients with behavioral management strategies, including applied behavior analysis (ABA), to improve daily functioning and social skills. Administer medications accurately following HIPAA guidelines and medical documentation standards, maintaining detailed medical records. Collect and analyze data related to client progress, behavior patterns, and treatment outcomes to inform ongoing care adjustments. Collaborate with families, caregivers, social workers, and healthcare professionals to coordinate services and ensure comprehensive support for each individual. Skills Experience working with children and adolescents facing mental health or developmental challenges. Proficiency in crisis intervention, crisis management, and de-escalation techniques to handle emergency situations confidently. Knowledge of working with individuals with autism spectrum disorder (ASD), disabilities, or developmental delays; familiarity with special education principles is a plus. Ability to perform vital signs measurement and monitor physical health indicators as part of holistic patient care. Strong understanding of social work principles, patient care standards, medical terminology, and medical records management. Skilled in intake processes, behavioral health assessments, data collection, and documentation compliance under HIPAA regulations. Competence in medication administration procedures while adhering to safety protocols and legal requirements.
