Job Description:

The role you'll contribute: Assists patients with activities of daily living (ADL) and routine tasks. Provides direct patient care and therapeutic interventions. Observes and records patient behaviors and activities accurately. Reports changes in patient conditions to appropriate personnel and recommend treatment plan modifications. Establishes and maintains therapeutic relationships with assigned patients. Collaborates with patients, family members, physicians, and community providers to ensure continuous care.
Knowledge, Skills, and Abilities:
Ability to communicate appropriately, professionally, and effectively with patients and their family, hospital staff, nurses, physicians, and other health care professionals Knowledge of commonly used concepts, practices, and procedures in the health care industry Up-to-date knowledge of regulatory requirements and local, state, and national laws Ability to understand and process instructions from physicians and other personnel Proficiency in computer systems to enter and track patient records
Education:
Associate [Preferred] High School Grad or Equiv [Required] Technical/Vocational School in a social science [Required]
Field of Study:
High School Grad or Equiv Required Work Experience:
1+ of other health care related field [Preferred]
