Job Description
St. Bernard Hospital is seeking a Mental Health Worker for our Mental Health Services department. The ideal candidate should be resilient, capable of crisis resolution, a team player, and dedicated to providing optimal care to patients. Required Education:
High school diploma or GED or the combination of education, knowledge, skills, mental development, and relevant experience that enables performance of all aspects of the position. Associates degree in Psychology, Social Worker or related field. (Preferred)Required Experience:
Minimum of two (2) years of experience in an acute psychiatric setting. Minimum of two (2) years of experience in an acute psychiatric unit in a health care setting. (Preferred)Duties & Responsibilities:
Take vital signs such as temperature, pulse, respiration, blood pressure, etc. Report any abnormal findings to the Charge Nurse. Record the findings in the Patient Care Record. Perform basic care as instructed by the Registered Nurse.Such duties include:
weigh and measure patient. Collect and assist with the collection of urine, sputum, and stool. Label all specimens for the laboratory and clock-in the time on the lab slip and place them in the appropriate area. Deliver samples within 15 minutes if stat venipuncture. Consistently apply and adhere to isolation procedures and techniques. Bath, dress and assist patients in oral hygiene, along with providing proper hygiene and skin care. Assist patients in and out of bed for purposes such as the bathroom; to sit in chair; or to walk around. Ensure patients receive their dietary trays and set-up trays for the patients. Assist and/or feed patients, as needed. Pick-up all dietary trays not picked up by Food & Nutritional Staff. Document on the Patient Care Record and issues with the patient's appetite. Inform the Registered Nurse of patient's appetites. Assist with the positioning and re-positioning of patients as instructed by the Registered Nurse demonstrating the importance of correct positioning and body alignment while utilizing comfort devices. Ensure patient beds are in the low position and locked at all times. Ensure that side rails are up at all times. Ensure patients are properly secured when transferring to or from a bed, wheelchair, or stretcher. When transferring patients, collect the patient's personal belongings; assist patient to wheelchair or stretcher; and orientate patient to new surroundings. Transport patients to and from other departments. Assist with admission, transfer, and discharge of patients. Report discharge planning needs to the nurse and social worker. Assist in the development, implementation, evaluation, and updating of individualized treatment plans for the adult and geriatric patients (age 18+). Observe patients' behavior, symptoms, patterns and deviations and report findings to nurse or supervisor. Observe and document in the progress notes patients' reactions to drug and treatment, or any other usual conditions and report adverse reactions to medication or treatment to medical personnel in charge. Assist in psychiatric emergencies and milieu management. Monitor patients on precautions/restraints according to hospital policy. Assist with individual counseling and group sessions to assigned patients. Complete daily progress notes using the D.A.R.P. format for assigned patients. Document on the Diagnostic and Dispositional Staffing form for primary patients. Maintain awareness of comfort and safety needs of patients. Remove harmful objects from patients. Reinforce no smoking policy. Assist patients in maintaining clean and neat rooms. Report the need for repairs promptly. Answer nurse call system promptly and provide prompt appropriate response to patient requests 100% of the time. Notify nurse immediately if need professional nurse intervention. Make frequent rounds to check on patients. Handle telephone information with courtesy, accuracy, and confidentiality. Identify patient and family teaching needs and consistently contribute to patient and/or family teaching. Communicate effectively with patients and family members and document conversations and teachings in the Patient Care Plan. Participate in department or unit quality improvement activities. Advocate and support unit and hospital activities. Achieve and maintain competency in skills and comply with policies associated with the skills inventory.
