Position would be working one on one in the community with an individual that has special needs.

Hours are as follows:
Position
  • Part Time
  • Monday
  • Friday 9am
  • 4pm.
PRN staff also needed. Interested applicants must have valid driver's license, automobile insurance, high school diploma/GED, and able to pass background checks.
Position Summary:
NC Innovations (I/DD) program is a Medicaid Waiver Program that trains and supports people with Intellectual and/or Developmental Disability. The services under the waiver help individuals learn skills to function more independently. The employee works directly with participants, parents, and other caregivers (individual or groups) in a naturally occurring setting (home, school, community, etc) to address functional problems that occur within that setting.
General Purpose:
Employees strive to develop the participant's ability to live successfully in the community with minimal services, support and guidance.
Role Qualifications:
Must execute program design to meet participant's needs and objectives Must partner with management team to ensure effective delivery of services Position responsibilities: § Completion of duties and the implementation of various skill building activities per participant's individualized Plan of Care (ISP) § Directs and assist individuals based on goals, task analysis and treatment team recommendations § Implement therapeutic monitoring and training as it relates to behavioral, educational, recreational and or social needs § Documentation submitted into CCW daily or within 48 hours of service delivery § Complete Incident reports per occurrence in CCW and within 24 hours of the incident § Attendance and participation at all supervisions, meetings, trainings, in-services or groups § Representation of agency in a positive manner at all times § Submit timesheet weekly and prior to 8:30am on Monday's if it is a holiday timesheets should be submitted prior to the holiday. § Participation in meetings for participant care and provide the best possible care at all times § Maintenance of skills, certifications and privileging § Uphold integrity of company and follows policies and procedures § Reports to QP any concerns or problems either employee or participant related § Adhere to other duties as assigned. Essential skills and experience (Qualifications): Have a high school diploma or GED. Be able to demonstrate competence in the area of confidentiality, participant rights, and strong documentation skills. Have strong organizational skills. Be 18 or older, have and maintain a valid NC driver's license and maintain liability insurance for their automobile. Be able to work in a non
  • supervised setting.
Must have reliable transportation. Have no findings on the healthcare registry.
