Music Therapist to provide music therapy consultations and services to the patient and/or family in accordance with the interdisciplinary plan of care while following all hospice policies and procedures. Support the business development activities of the organization. Provide bereavement counseling to family members as identified.

Brief Job Responsibilities:

Complete a music therapy assessment with patient and family when referred in accordance with the interdisciplinary plan of care. Implement the music therapy interventions in accordance with the interdisciplinary plan of care. Guide the patient in the use of creative therapeutic activities for the purpose of improving or maintaining mental and physical function and quality of life. Instruct other care team members including, when appropriate, hospice aides and family members in the use of music to achieve individualized care plan goals. Provides consultation, education, and support to the interdisciplinary group on music therapy services. Provides for funeral or memorial services for patients as requested. Actively participates in quality assessment performance improvement teams and activities. Position Qualifications 1. Bachelor's Degree in Music with a concentration in Music Therapy and/or Music Therapy. 2. Board Certified as a Music Therapist by the Certification Board for Music Therapy. 3. Experience in hospice care is preferred. 4. Hospice and Palliative Care Music Therapist preferred. 5. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.. 6.
