Job Description
Qualifications
- LCSW, LCSWA, LCMHC, LCMHCA, LMFT, or LMFTA
- Must be licensed in the state of North Carolina Responsibilities
- Conducts ongoing evaluation, assessment, counseling, and treatment to children/families in need of services
- Work closely with the person-served physician, other clinical staff, and community agencies
- The Licensed In-Home Therapist applies principles of Evidence Based Practice to all services provided and ensures team members maintain fidelity to the treatment model
- Conducts weekly meetings with the Intensive In Home team to provide supervision, monitor implementation of the Person Centered Plan, manage productivity, and monitor the overall performance of the team
- Monitors the team to family ratio to ensure compliance with Medicaid service definition requirements The Licensed Therapist/team lead will reviews documentation of team members and ensure all notes are submitted within a timely manner, meet Medicaid requirements, and address the goals as written in the Person Centered Plan
- The Licensed In-Home Therapist will provide assistance with crisis management, as well as individual therapy and individual counseling
