This nurse practitioner job is located onsite at Hillview Terrace at 100 Perry Hill Road Montgomery, Al 36109 Company Background Primary Care Select is dedicated to improving patient care and patient outcomes in the Skilled Nursing and Long-Term Care Nursing Facility residential environments. Our unique model provides responsive onsite medical coverage to the patients and residents of our sister facilities to extend the reach of our Medical Directors and attending physicians. Working together with these facilities and their staff, Primary Care Select is making a dramatic difference by enhancing the speed of patient rehabilitation progress, improving overall clinical outcomes, and lowering rates of unnecessary hospitalizations. Nurse Practitioner Job Description Working collaboratively as a key member of the medical staff, the NP plays a critical role in managing the medical needs of Skilled Nursing Facility patients and Long-Term Care residents. Primary Care Select NPs build strong working relationships with Facility Administrators, Directors of Nursing, and other staff within their designated facility, functioning as the primary Provider point of contact for acute care and other medical needs. Primary Care Select trains our NPs to work relatively independently to see patients and treat complex medical conditions in an environment that needs and appreciates you. This collaborative independence leads to very fulfilling work. Primary Care Select provides a highly competitive salary and other benefits. Nurse Practitioner Responsibilities
- Provide Post-Acute medical services to Skilled Nursing Facility patients recently discharged from local hospitals. Oversee the rehabilitation process and plan and prepare patients and families for eventual patient discharge.
- Provide primary, urgent, and preventive care to Long-Term Care residents.
- May provide similar care to local Assisted Living Facility residents.
- Verify admission orders and conduct initial assessments on new Skilled patients and newly admitted Long-Term residents.
- Conduct Physical exams, medication reviews, and medical screenings.
- Collaborate with the Facility Medical Director, attending physicians, and nursing staff to deliver the highest level of care possible to all patients in the Nursing Facility.
- Communicate with patients, residents, and family members regarding changes in clinical status, treatment options, and prognosis.
