Join our Mesa, AZ team as a full-time provider delivering comprehensive primary care to adults and older adults. You will manage acute and chronic conditions, coordinate patient-centered care, and work collaboratively to improve health outcomes for a diverse patient population. The ideal candidate demonstrates strong clinical judgment, cultural sensitivity, excellent communication, and a commitment to continuous learning and compassionate care.

Requirements:
Graduate of an accredited Nurse Practitioner program Arizona Nurse Practitioner certification and valid Registered Nurse license Arizona Advanced Practice license Active DEA registration Current Basic Life Support (BLS); CPR/AED certification per AHA or Red Cross Level One Fingerprint Clearance Card (required for behavioral health sites) Valid Arizona driver's license Experience treating adults with acute and chronic conditions; geriatrics experience or work with patients with multiple chronic illnesses preferred Proficiency with clinical assessment methods and electronic health record systems Strong interpersonal and communication skills; ability to work independently and make sound clinical decisions Professional written communication skills (business English, spelling, punctuation) Demonstrated commitment to patient-centered care and ongoing professional development
