Patient homes and community-based settings, including assisted living and skilled nursing facilities Overview The Judge Group is seeking a Field-Based Advanced Practice Provider to deliver comprehensive primary care to patients in their homes and community-based settings throughout Maricopa County. The provider will serve as the patient's primary care provider of record, manage chronic conditions, develop individualized treatment plans, and promote continuity of care. Providers typically see approximately five to six patients per day and manage a patient panel of fewer than 200 patients. Travel is required within an approximately 50-mile radius of the provider's home, with cluster scheduling used to support efficient travel. Planned administrative time is included during the week, with flexibility for the provider to select when that time is scheduled. Candidates located in or near Phoenix, Scottsdale, Tempe, Mesa, Chandler, or Gilbert are encouraged to apply. Key Responsibilities Conduct in-home visits, including assessments, examinations, and medication reviews. Complete approximately five to six patient visits per day, with a minimum expectation of 25 visits per week. Serve as the primary care provider of record for a patient panel of fewer than 200 patients. Manage chronic conditions and develop individualized treatment plans. Conduct appropriate patient follow-up visits. Maintain accurate and thorough EMR documentation. Manage the patient inbox promptly and effectively, with support from medical assistants and registered nurses. Educate patients on disease management and healthy living. Collaborate with interdisciplinary teams to ensure coordinated care. Maintain patient, team member, and employer confidentiality in compliance with HIPAA regulations. Participate in rotating on-call coverage approximately once every five weeks. Travel regularly to patient homes, healthcare facilities, and community locations. Required Qualifications Active, unrestricted provider license in Arizona that is clear of clinical or board practice restrictions. Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant. At least one year of experience in primary care or a related field, such as family practice, general practice, or outpatient care. Ability to obtain autonomous provider status within six months of meeting Arizona eligibility requirements. Strong understanding of CMS and Affordable Care Act risk-adjustment models. Demonstrated clinical skills necessary to perform in-home assessments. Ability to work independently and make a meaningful impact on patient health. Ability to document assessment findings accurately and thoroughly. Ability to successfully complete required background checks and a 10-panel drug screening. Reliable transportation and a clean driving record. High-speed cable or fiber-optic internet with a minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds before starting. Ability to use computer and phone systems frequently. Ability to lift and carry up to 20 pounds. Ability to travel in varying weather conditions, road conditions, and unfamiliar environments. Preferred Qualifications Nurse Practitioner with autonomous practice status. Two or more years of experience in primary care, family medicine, adult care, longitudinal care, or outpatient care. Three to five years of relevant clinical experience is strongly preferred. Significant registered nurse experience may be considered alongside primary care or outpatient experience. Certifications/Licensure Active, unrestricted Arizona provider license. Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant certification. Active DEA license.
