Full Job Description Job Purpose:
At Denova, our Family Nurse Practitioners (FNPs) are at the forefront of integrated healthcare, delivering comprehensive, family-centered care across all stages of life. As a key member of our team, you will work autonomously and alongside other healthcare professionals, utilizing a whole-person approach that seamlessly blends primary care, behavioral health, addiction medicine, and wellness strategies.What You Will Do:
Comprehensive Care Management:
Develop treatment plans for acute and chronic conditions, focusing on holistic patient outcomes.Preventive Health Education:
Guide patients on disease prevention and healthy lifestyle choices to foster long-term wellness.Lifecycle Health Promotion:
Adapt health strategies to accommodate the aging process.Diagnostic and Evaluation Expertise:
Perform thorough exams and evaluations to diagnose and monitor health concerns.Prescriptive Authority:
Utilize your expertise to prescribe medications effectively and safely.Leadership and Independence:
Lead healthcare initiatives and manage patient care with autonomy. What We Need F romYou:
Master's degree in nursing. Current Arizona Nurse Practitioner licensure Board certified Family Nurse Practitioner, with current DEA and ANCC certifications. Experience using electronic medical records. Current BLS and a valid Arizona Fingerprint Clearance card. 1-3 years of Family Practice, Primary Care, or related Nurse Practitioner experience. Prior experience in an integrated care setting is a plus Strong clinical assessment, communication, and time management skills. Ability to work independently while also collaborating effectively within a multidisciplinary team.Your Work Schedule:
Part Time onsite position 2 10-hour shifts per week 7:00 AM - 5:00 PM One scheduled day must be eitherMonday or Friday Perks of Being Part of Denova:
