Position Summary:
The FNP is an advanced practice nurse that performs all aspects of patient care, including diagnosis, treatments and consultations. He/she will work in outpatient situations and can perform independently or as part of a treatment team. FNF must be prepared to work with patients who may be uncooperative, disoriented or aggressive. The FNP must also be able to meet the physical demands of the job, including spending significant periods of time on his/her feet, as well as being immersed in a potentially stressful environment.Duties and Responsibilities:
Effectively identifies, evaluates, addresses and treats disease and prevention and health promotion issues while administering quality patient care. Examines a patient and diagnoses his/her condition(s). Studies and interprets the tests and records of the patient. Discusses and formulates proper and personalized health care plans for patients. Prescribes varied forms of medications and other treatment methods to the patients. Advise patients to seek specialized medical help, if the treatment requires that. Answers the queries that a patient might have about patient's medical condition. Completes special forms within CHA's scope of practice. Attends meetings and huddles with CHA treatment teams and licensed staff. Perform special assignments, projects, and other duties as assigned. Ability to work in a fast paced and demanding environment. Meet physical demands of the job such as spending significant periods of time on their feet and sitting as well as immersed in a potentially stressful environment. Minimum Qualification toPerform Job:
Master's degree in nursing Licensed as a Nurse Practitioner in the State of Arizona required. Must be 21 years of age. Exceptional skills in communication both written and verbal, conflict resolution and client relation. Must be able to read and interpret documents and have the ability to develop routine reports and correspondence. Understand the role of the agency in the total community welfare plan, and to interpret in-depth and imaginatively casework services and the agency's function. Carry important and continuing assignments in community works. Proficiency with personal computers and applicable software programs. Maintains current license and certificate(s) required by Office of Medical Licensure to hold position. Valid Driver's License and clean driving record. Level one fingerprint clearance card.Skills:
Excellent communication skills and bedside manners, including ability to respond to callers/members with patience, objectivity and a nonjudgmental attitude. An ability to prioritize tasks and allocate time effectively, ensuring each patient receives the attention and care they need without feeling rushed. Committed to empowering patients through clear and informative education, helping them make informed decisions about their health and treatment options. Demonstrates genuine empathy and kindness, fostering a trusting relationship with patients. This approach not only supports their emotional well-being but also encourages them to actively engage in their wellness journey.Conditions and Equipment:
Working conditions include office setting as well as programming specific to population and customers. Fast paced and demanding supervision of multiple programs and services. This person will be active in supervising a variety of programs in a variety of settings with some travel necessary.
