In this vital role, you will provide comprehensive primary care services, perform advanced clinical assessments, and develop personalized treatment plans for a diverse patient population across all ages. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions in an outpatient clinic. Responsibilities:

Conduct Thorough patient assessments, including vital signs, physical examinations, and medical History reviews, to determine health status. Diagnose and treat acute illnesses and chronic conditions using evidence-based practices. Prescribe Medications in accordance with state regulations. Manage electronic records EHR/EHR system and ensure seamless medical documentation for quality assurance

Qualifications:

Valid state licensure as a Family Nurse Practitioner with current certification from the American Association of Nurse Practitioners Must be bilingual (English/Spanish) Demonstrated experience working in primary care settings with proficiency in a medical office Strong clinical skills including physical examinations ,diagnostic evaluation. Experience working with diverse populations, including pediatrics, geriatrics, and individuals with disabilities. Familiarity with EMR/EHR systems.
