Full Job Description Job Summary:

We are seeking a compassionate and highly skilled Family Nurse Practitioner to join our healthcare team. As a Family Nurse Practitioner, you will work collaboratively with other healthcare professionals to diagnose, treat, and manage a wide range of acute and chronic conditions. Duties:

• Conduct comprehensive physical exams, including assessing patients' vital signs and medical histories.
• Order and interpret laboratory tests and imaging studies as necessary to diagnose and manage patient conditions.
• Develop and implement individualized treatment plans for patients of all ages, from newborns to geriatric patients.
• Provide preventive care, including routine check-ups, health screenings, and vaccinations.
• Manage chronic conditions, such as diabetes, hypertension, and asthma, through medication management, lifestyle counseling, and patient education.
• Collaborate with healthcare teams to coordinate patient care, including referrals to specialists and other healthcare services.
• Utilize telehealth technology to provide virtual consultations and monitor patient progress remotely.
• Stay current with the latest developments in medicine and healthcare policy through ongoing education and training.
• Participate in case management to optimize patient outcomes and reduce healthcare costs.
• Provide occupational health services, including conducting physical exams and providing health education to employees.

Experience:

• Minimum of a Master's degree in Nursing (MSN)
• Current licensure as a Family Nurse Practitioner in California
• Current DEA License in California
• Certification as a Family Nurse Practitioner (FNP) or the equivalent.
• A minimum of 2 years of experience as a Family Nurse Practitioner or in a related role.
• Experience providing telehealth services to patients.
• Experience working in a team-based healthcare environment.
• Bilingual, preferably Spanish.
