In this position you will provide high-quality, patient-centered care to a diverse adult population ranging from acutely ill inpatients to long-term post-acute and custodial residents. Your clinical expertise will directly improve patient outcomes through comprehensive assessments, evidence-based management, and close care coordination. You will have extensive physician support and work within a healthy, collegial environment that values clinician wellbeing andprofessional growth. Duties •Round daily on assigned patients across acute care, skilled nursing, and assisted living settings, adapting your clinical approach to the acuity and care level of each environment. Conduct thorough adult patient assessments, including history, physical examination, and vital-sign interpretation. • Develop and implement individualized treatment plans in collaboration with attending internal medicine physicians. Manage acute and chronic medical conditions — hypertension, diabetes, heart failure, COPD, CKD, dementia, infections, and the full spectrum of adult internal medicine problems — using evidence-based practice. Perform inpatient admissions, daily progress evaluations, change-of-condition visits, and discharge or transition planning. Deliver chronic care management and longitudinal primary-style oversight for assisted-living residents, coordinating with facility staff, families, and specialists. Perform and manage bedside clinical tasks appropriate to the setting, including medication management, wound care, and IV therapy oversight. Document encounters accurately and efficiently in the EHR and ensure coding and billing appropriately capture the care delivered. Provide care transitions between hospital, SNF, and ALF settings to promote continuity and reduce avoidable readmissions. Engage in patient and family education on diagnosis, medications, advance care planning, and goals of care. Collaborate closely with attending physicians on complex cases, with real-time support available by phone, text, or in person throughout the day. Requirements Current DEA registration and board certification. Active, unrestricted Nurse Practitioner license in the state of practice (FNP, AGNP, or AGACNP); adult-gerontology certification strongly preferred. Demonstrated ability to work adaptably across multiple clinical settings — comfortable shifting between acute care, post-acute, and custodialenvironments in the same day. Experience in hospital medicine, post-acute / long-term care, or geriatrics strongly preferred; motivated new grads with strong clinical reasoning and a genuine interest in this patient population will beconsidered. Critical care, hospitalist, or emergency-medicine experience is a plus. Proficiency with EMR/EHR systems; working knowledge of ICD-10 and CPT coding for inpatient E&M, chronic care management, and post-acute visits. Strong communication and interpersonal skills for effective collaboration with multidisciplinary teams, facility staff, specialists, patients, and families. Self-directed — able to manage a rounding list independently while knowing when to loop in physician support. Reliable transportation for travel between facilities.
