In this vital role, you will provide comprehensive primary care services to diverse patient populations, including pediatric, adult, and geriatric patients. Your expertise will help deliver high-quality, patient-centered care in a fast-paced environment, ensuring each individual receives personalized treatment and education. Responsibilities Conduct thorough physical examinations and patient assessments across various age groups, including children and seniors. Manage acute and chronic conditions utilizing evidence-based practices, including acute pain management and infectious disease care. Administer injections such as immunizations, dermal fillers, botulinum toxin treatments, and IV infusions with precision and safety. Perform diagnostic evaluations using medical imaging techniques like sonography or laboratory specimen collection/processing when necessary. Document all patient encounters accurately within electronic health record (EHR) systems such as Elation EMR, ensuring compliance with HIPAA regulations. Provide triage services via telehealth or in-person consultations to prioritize urgent cases effectively. Assist with procedures including suturing, catheterization, spinal taps, or tube feeding as required for patient care. Collaborate with multidisciplinary teams on discharge planning, case management, or utilization review for optimal patient outcomes. Maintain sterile techniques during procedures like sterile processing or aseptic technique applications in clinical settings. Support patients with specialized needs such as assisted living residents, hospice & palliative medicine cases, or individuals with developmental disabilities. Qualifications Current Nurse Practitioner license with family practice certification; prior experience in primary care preferred. Demonstrated proficiency with EMR/EHR systems such as Elation EMR, or; telehealth experience is a plus. Strong clinical skills including physical examinations, medication administration, venipuncture, and injection techniques. Knowledge of medical terminology, anatomy knowledge, physiology principles, and diagnostic evaluation methods. Experience working in diverse settings such as urgent care, hospital medicine (including ICU or Level I Trauma Centers), nursing homes, or outpatient clinics. Familiarity with CPT coding and ICD coding systems for accurate documentation and billing purposes. Ability to handle multiple priorities efficiently while maintaining a lively pace; critical care experience is advantageous. Excellent communication skills with the ability to work effectively with children, seniors, and individuals with disabilities or complex health needs.
