Job Description

Overview Join our dynamic Wound Care Clinic as a Nurse Practitioner dedicated to transforming patient outcomes through expert wound management and comprehensive care. In this vital role, you will utilize your advanced clinical skills to assess, treat, and educate patients with complex wounds, ensuring optimal healing and quality of life. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and detailed wound evaluations using advanced clinical techniques. Develop and implement individualized treatment plans incorporating wound debridement, dressing changes, and advanced therapies. Perform specialized procedures. Utilize electronic health record (EHR) systems to document patient encounters accurately and efficiently. Collaborate with multidisciplinary teams across specialties such as infectious disease care, geriatrics, hospice & palliative medicine, and acute care to coordinate comprehensive patient management. Educate patients and their families on wound care procedures, medication administration, nutrition, and lifestyle modifications to promote healing and prevent complications. Experience Proven experience as a Nurse Practitioner in wound care or related fields such as hospital medicine, emergency medicine, or critical care environments. Demonstrated proficiency with EMR/EHR systems for medical documentation and case management. Hands-on experience performing advanced procedures and sterile processing techniques. Familiarity with CPT coding, ICD-10/ICD-9 coding systems for accurate billing and documentation. Background working with diverse populations such as pediatrics, geriatrics, hospice patients, or those in assisted living or nursing home settings. Knowledge of infection control standards and infection prevention practices within acute care or outpatient clinics.
