Job Description

We are a growing Physician Owned Physical Medicine & Rehabilitation (PM&R) and Pain Medicine practice seeking a motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a Skilled Nursing Facility (SNF) setting. This is an excellent opportunity for providers looking to expand their procedural skillset, develop ultrasound expertise, and work in a flexible, high-growth environment with strong earning potential. Locations This role involves rounding at skilled nursing facilities across the following areas: Lodi, California Pacifica, California Stockton, California Facility assignments are geographically clustered to minimize travel. What We Offer Flexible scheduling - full-time or part-time options High earning potential through a productivity/collections-based compensation model Comprehensive procedural and ultrasound training Autonomy in practice with strong physician support Geographic flexibility with clustered facility assignments Key Responsibilities Evaluate and manage patients within skilled nursing facilities Diagnose and treat conditions including: Functional decline Musculoskeletal pain Spasticity Neuropathy Post-hospital rehabilitation needs Perform bedside procedures Conduct ultrasound-guided injections Collaborate with interdisciplinary teams (PT/OT/ST) Maintain accurate and timely EMR documentation Work closely with supervising PM&R and Pain Medicine physicians Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) Minimum of 2 years of experience as a mid-level provider (required)
Required:
Prior experience in PM&R and/or Pain Medicine Active DEA Registration (required) Strong clinical and diagnostic skills in a SNF or similar setting Ability to thrive in a fast-paced, high-volume environment Strong interpersonal and communication skills Procedural experience or strong interest in procedures SNF experience is a plus Compensation & Engagement Structure 1099 independent contractor position Productivity/collections-based compensation structure High income potential driven by patient volume and efficiency Paid training and ongoing clinical development Flexible scheduling options Must maintain active personal malpractice/liability insurance Training & Development We provide structured, hands-on training in: Ultrasound fundamentals Ultrasound-guided injections Trigger point and joint injections Spasticity management SNF rehabilitation workflows Documentation optimization Schedule Options 1-2 days per week minimum Part-time or full-time opportunities Flexible rounding schedules Geographic clustering to minimize travel Why Join Us?
