POSITION SUMMARY

The Primary Care Provider (NP) has a specialized focus on perinatal and women's health. This includes preventive care, diagnosis, and treatment of various health conditions specific to gynecology, including reproductive health, pregnancy, menopause, gender affirming care, and general wellness. The provider will work collaboratively with other healthcare professionals to ensure each patient receives holistic and individualized care. The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health (AAAHC) and the Commission of Dental Accreditation (CODA).

CORE RESPONSIBILITIES 1.

Clinical Care:

Provide comprehensive primary care with emphasis on women's health throughout the lifespan 2. Perform preventive screenings, routine gynecological procedures including Pap smears, and health maintenance 3. Manage acute and chronic conditions with attention to gender-specific presentations 4. Deliver reproductive health services, including contraception counseling, gender affirming care, and LARC procedures including IUD and Nexplanon 5. Address women's health concerns including menstrual disorders, menopause, and sexual health 6. Implement evidence-based treatment plans incorporating physical, mental, and social health factors 7.

Team Leadership & Collaboration:

Lead daily huddles with complex care team including RNs, care coordinators, and specialists 8. Champion NAHC's integrated care model through effective team leadership 9. Mentor and develop Medical Assistants and clinical support staff 10. Participate in interdisciplinary care planning and case conferences 11. Maintain productive relationships with internal and external healthcare partners 12.

Quality & Compliance:

Complete clinical documentation within 72 hours of patient visits 13. Process medication refills and authorization requests within 5 business days 14. Complete patient medical forms within 10 business days 15. Meet productivity standard of 18-20 patients per day 16. Fulfill mandatory reporting requirements for births, deaths, and reportable conditions 17. Maintain compliance with HIPAA regulations and clinic policies 18. Participate in quality improvement initiatives and stay current with medical research 19.

Patient Education & Community Engagement:

Provide culturally-sensitive health education to patients and families 20. Develop and implement health education programs 21. Participate in community health initiatives 22. Consider cultural, psychological, and environmental factors in treatment planning. 23.

Safety:

Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves. 24.

Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention. 25.

HIPAA:

Keep all protected health information (PHI) confidential and abide by 26. HIPAA guidelines for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to Site Director and HIPAA security officer. 27.

HIPAA:

Ensure the protection of all PHI and compliance of HIPAA guidelines amongst staff under your supervision. Take appropriate steps to address HIPAA concerns with staff as necessary. 28. Works extremely well under pressure, meet multiple and often competing deadlines. 29. At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community. 30. Other duties as assigned by Supervisor.

MINIMUM QUALIFICATIONS 1.

Must be a licensed FNP in the state of California. 2. Must be Board Certified in WFNP, Family Practice, Women's Health or other full scope Primary Care Specialty. 3. Current DEA license. 4. Three (3) years of experience working within a community clinic. 5. Must be able to provide TB and Physical clearance prior to start date. 6. CPR certification must be obtained within 90 days and kept current at all times. 7. Meets credentialing requirements established by the Native American Health Center. 8. Must have a National Provider Identification (NPI), BLS, DEA and depending on specialty ACLS or NALS or PALS or PEARS may also be required. 9. Able to furnish Vaccination history prior to start date dependent on role. 10. Excellent communication and written skills. 11. Ability to work independently and as part of a multi-disciplinary team. 12. Ability to work well under pressure, analyze and evaluate individual member's needs, reach sound conclusions and make appropriate recommendations. 13. Must be detail oriented and be able to multi-task effectively. 13. Previous experience with an Electronic Health Records (EHR) system and working in an EHR environment is desired. 14. Commitment to Native American Health Center's values and mission. 15. Knowledge of the Bay Area Native American community with a commitment to serving Native Americans and other vulnerable member populations. 16. Must have ability to relate and communicate effectively with community interest groups, agencies, and medical facilities that serve the American Indian community. 17. Work extremely well under pressure, meet multiple and often competing deadlines. 18. Must be able to meet set priorities as directed by supervisor. 19. Must be detail oriented and a team player.

PREFERRED QUALIFICATIONS 20.

Two to five (2-5) years of experience working with a community clinic is highly desirable. 21. Excellent communication and written skills. 22. Established leadership skills. 23. Bilingual in Spanish. 24. Experience with

system is strongly desired.
