As Circadia's home health NP, you'll see patients first in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions-reducing preventable hospitalizations. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting. What You'll Do Direct patient care: Conduct initial and follow-up in-home assessments on flagged patients; initiate/adjust plans of care; complete consent; coordinate with referring and primary care providers.

Tech - bedside integration:

Train and coach caregivers and home health aides at the point of care, standardize use of Circadia into home health clinical workflows Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.

Close the loop:

Complete documentation/charts within the same day Home-based visit management: Efficiently complete 6-8 in-home visits per day across designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts. Quality & product feedback: Surface field insights to improve workflows and tech integration in a home health setting Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.

Regional flexibility:

Cover sites within a 30-40-mile radius; for sites outside of that radius, mileage and lodging covered per policy. How Success is Measured (KPIs)

Visits:

Initially targeting around 6 days per month, with flexibility to increase over time (10+ days)

Documentation:

Complete all notes using AI-powered tools within 24 hours of each encounter. Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time. Must-Have Qualifications Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. Eligible for Medicare participation; company bills on your behalf. 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently. Valid CA driver's license; reliable transportation for in-home visits across the SoCal/OC area. Comfortable conducting face-to-face visits in a home environment, including with patients requiring complex care. Thrives in a dynamic, evolving startup environment. English required.
