Palo Alto Foundation Medical Group is seeking a part-time, certified Nurse Practitioner or Physician Assistant in Palliative Care.Location:

Managing health problems and coordinating health care for the patient in accordance with State Federal Rules and Regulations Provide comprehensive, coordinated, and compassionate care for patients and families facing serious illness Formulates a comprehensive care plan, patient education, and ongoing evaluation in collaboration with the palliative care interdisciplinary team for palliative care patients in the home, office and SNF settings Prescribes and administers medications/immunizations including controlled substances to the extent delegated and licensed, and in collaboration with palliative care and/or referring physicianQualifications:
Requirements:
6 months related clinical experience in Oncology, Geriatrics, Hospice or Palliative Care Current California State NP or PA License CAPA Controlled Substance Certification (for Physician Assistants only) Current Nurse Practitioner Furnishing Number (for Nurse Practitioners only) Nurse Practitioner furnishing number of NP's Active DEA registrationOrganization Details:
