As Circadia's on-site NP, you'll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. This is a full-time, embedded NP role requiring Monday-Friday, on-site presence at our partner facility, Mirage Post-Acute in Lancaster, CA. What You'll Do Direct patient care: Conduct initial and follow-up in-home assessments on flagged patients; initiate/adjust plans of care; complete consent; coordinate with referring and primary care providers. Tech → bedside integration: Train and coach caregivers and home health aides at the point of care, standardize use of Circadia into home health clinical workflows Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.

Must-Have Qualifications Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. Eligible for Medicare participation; company bills on your behalf. 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently. Valid CA driver's license; reliable transportation for in-home visits across the SoCal/OC area. Thrives in a dynamic, evolving startup environment. English required.
