This energetic position offers the opportunity to work collaboratively with a multidisciplinary team to improve men's health outcomes through innovative treatment plans, including hormone therapy, sexual health optimization, and wellness assessments. You will utilize your clinical expertise to evaluate, diagnose, and manage a diverse patient population in a fast-paced, supportive environment committed to excellence and continuous improvement. Duties Conduct thorough patient assessments including medical history review, physical examinations, and vital signs measurement to determine health status and treatment needs. Prescribe medications and develop personalized treatment plans for hormone therapy, erectile dysfunction, and other men's health concerns in accordance with collaborative practice agreements and DEA regulations. Administer injections such as testosterone or botulinum toxin treatments, ensuring proper technique and patient safety. Utilize EMR/EHR systems like Epic or Athenahealth for accurate documentation of patient encounters, treatment plans, and medical records management. Educate patients on health maintenance strategies, lifestyle modifications, and medication adherence to promote optimal wellness. Collaborate closely with healthcare professionals including specialists in urology, endocrinology, and behavioral health to coordinate comprehensive care. Stay current with advancements in clinical research related to men's health, hormone therapies, and esthetic laser treatments to provide evidence-based care. Experience Proven experience working with electronic health record (EHR) systems such as Epic or Athenahealth is essential. Demonstrated proficiency in performing procedures like IV infusion, venipuncture, suturing, catheterization, and sterile processing techniques. Prior experience administering botulinum toxin treatments or dermal fillers is highly desirable. Background in critical care settings such as ICU or emergency medicine enhances ability to manage acute situations effectively. Knowledge of CPT coding for billing purposes along with ICD-10 coding standards is required for accurate documentation. Experience working within collaborative practice agreements as an advanced practice provider (APP) in outpatient or clinic settings is preferred. Familiarity with telehealth platforms to deliver remote consultations aligns with our commitment to accessible care. Led by Dr. Rommie Fakhoury, a board-certified physician with 25+ years of experience, our Rancho Cucamonga clinic offers personalized, discreet, provider-supervised care in testosterone replacement therapy (TRT), erectile dysfunction treatment, medical weight loss, and men's wellness. Our patients don't come to us because they're sick — they come to us because they want to feel great again, and that makes this some of the most rewarding clinical work you'll do. We're growing, and we're looking for a sharp, compassionate PA or NP who wants real clinical autonomy, a predictable schedule, and the satisfaction of watching patients transform week over week. What You'll Do Evaluate new and returning patients for low testosterone, ED, and related hormonal/wellness concerns Review labs and patient history to build individualized, results-driven treatment plans Prescribe and manage testosterone therapy and related medications in collaboration with our medical director Monitor progress and fine-tune dosing to keep patients optimized, not just "in range" Educate and coach patients through their treatment journey — this is relationship-driven medicine Support ancillary services including medical weight loss (GLP-1/semaglutide, tirzepatide), peptide therapy, and NAD+ Maintain accurate, compliant clinical documentation Work closely with a small, tight-knit front office and clinical team to deliver a seamless, discreet patient experience What We're Looking For Active California PA-C or NP license (or license-eligible) National certification (NCCPA for PAs;
AANP/ANCC
