Nurse Practitioner or Physician Assistant (Primary care)
Location:
Salinas, CA Job Type:
Full-time Shift:
Monday•Friday, 8 am•5 pm
Work Location:
In person.
Pay Range:
$130k•160k
About Your Work:
Obtain history and perform physical examination Complete review of medicines and related instructions Review home safety and psychosocial needs Fulfill documentation needs for STAR and HCC measures Obtain and document all pertinent information in patient's charts and case management notes on the same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions.
Qualifications:
CA Licensed Nurse Practitioner 1+ Experience with
